Required Forms

A paper or digital Medical Release form (template) for all Players under 18 years of age must be kept with a coach or team chaperone at all times while attending the Volleyball Festival.

Bring a copy of your AES roster available at Team Check-in. For any AAU membership numbers in AES not showing as “verified,” bring one of the three following proofs of membership:

1. AAU Membership Cards for each athlete and non-athlete on the Team Roster; or
2. AAU Club Listing (generated from AAU “manage your club” website); or
3. AAU Team Roster (generated from AAU “manage your club” website)
